Even if you’ve successfully managed your type 2 diabetes (T2D) for years, you may begin to notice it’s harder to reach your blood sugar targets. While that might feel discouraging, T2D is considered a progressive condition, and it’s perfectly normal to tweak management strategies to more effectively fight the disease.

“Type 2 diabetes is considered progressive because the body’s ability to produce and use insulin changes over time,” says Barbara Eichorst, RD, CDCES, the vice president of healthcare programs at the American Diabetes Association. “Even with consistent self-care, the pancreas may gradually make less insulin, and the body may become more resistant to it.”

But you can make adjustments to your wellness approach to slow or stop disease progression as your T2D evolves.

How Type 2 Diabetes Progresses

T2D typically follows a set pattern of development from the earliest onset to end-stage complications, although effective treatment can slow or stop the pace.

  • Insulin Resistance This condition happens when your body’s cells stop responding effectively to insulin, a hormone that regulates blood sugar levels. It’s influenced by both genetic and lifestyle factors, and over time, insulin resistance can lead to prediabetes or diabetes.

  • Prediabetes This condition is marked by blood sugar levels that are higher than normal, but not yet high enough for a diagnosis of type 2 diabetes.

     It’s often reversible with weight loss, a healthier diet, and regular exercise.
  • Beta-Cell Burnout Beta cells are specialized cells in the pancreas that produce, store, and release insulin. When they stop working properly, the pancreas can no longer keep up, and blood sugar continues to rise, potentially leading to a T2D diagnosis.

     “The decline of beta cell function in T2D is greatly variable from patient to patient,” says Silvana Obici, MD, the chief of the division of endocrinology at Stony Brook University Renaissance School of Medicine and the medical director of the Stony Brook Medicine Diabetes Center in Lake Grove, New York.
  • Long-Term Progression Over years or decades, insulin-producing cells continue to decline, and you may need treatment with insulin injections or other advanced therapies. Chronic hyperglycemia can also lead to a host of complications, such as heart disease, eye problems, nerve damage, and foot sores.

Physiological Factors for Disease Progression

Eichorst says progression is driven by a combination of factors, including a gradual decline in insulin-producing beta cell function, increased insulin resistance in the liver and muscles, hormonal changes that affect blood glucose regulation, and chronic inflammation.

“Together, these changes make it more difficult for the body to regulate blood glucose effectively over time,” she says.

Lifestyle choices, heredity, and demographics also factor into how quickly the disease can emerge and ramp up.

  • Age The disease is more prevalent the older we get,

     but “adults over 60 progress more slowly than middle-aged individuals,” says Dr. Obici.
  • Ethnicity Communities of color are more likely to develop T2D and also more likely to suffer health complications from it, which researchers attribute largely to socioeconomic factors like disparities in income, housing, food environment, and healthcare access.

  • Body Weight Obesity is a leading contributor to the development and progression of type 2 diabetes.

  • Diet Both food insecurity and a poor-quality diet higher in refined grains, added sugar, sodium, and fat are associated with worse T2D control.

  • Lack of Exercise More time spent sitting has been linked to both an increased risk of type 2 diabetes and to cardiovascular complications if you already have diabetes.

  • Genetics Up to 70 percent of the risk for T2D can be attributed to genetic factors. Your diabetes may progress more quickly or slowly depending on your family history of the condition.

Other risk factors for the progression from prediabetes to type 2 diabetes include male sex, anxiety, depression, high blood pressure, and unhealthy levels of cholesterol and liver enzymes, according to a meta-analysis of almost five dozen studies.

How Treatment and Management May Change Over Time

Despite decades of research, it’s still unclear why some people need to make more adjustments to manage their diabetes than others. Some people are able to control their blood sugar with diet and exercise alone, some eventually need to take medication, and others may need to progress to multiple medications, including insulin.

 An honest conversation with your doctor about any new developments will allow them to tailor your treatment plan.
Beyond medication adjustments, lifestyle interventions are still one of the most essential tools for T2D management. Our bodies naturally accumulate body fat as we age, which has a powerful impact on disease progression.

Keeping an eye on your diet helps with both diabetes and weight management, and exercise is another great way to make some improvements. Research has consistently shown that physical activity, including aerobic exercise and strength training, can help control blood sugar levels and improve insulin resistance in people with T2D.

“Lifestyle interventions aimed at sustained weight loss of at least 7 percent of body weight and at least 150 minutes of moderate-intensity exercise weekly can delay or prevent progression of type 2 diabetes, as demonstrated by the National Diabetes Prevention Program,” says Obici.

When to Talk to Your Doctor

As is the case with all types of disease management, good communication is an important component of good health outcomes.

“It is important to stay connected with your healthcare team,” says Eichorst. “Ongoing, proactive conversations are key to staying ahead of changes.”

Eichorst says to consult your healthcare provider if your symptoms are changing in any of the following ways:

  • Your blood glucose levels are trending higher or becoming harder to manage.
  • You notice new symptoms like increased thirst, fatigue, or frequent urination.
  • You’re experiencing changes in weight, effects from the medications you take, or in your overall health.
  • You’re experiencing life transitions, such as job changes, relocation, or sleeping problems that may affect your routine.
