Research has linked pesticide exposure and cancer for decades. But the relationship between the two is complicated. Many factors influence a person’s risk of developing cancer, including the amount of pesticides they’re exposed to and how often it happens.

“‘The dose makes the poison’ is a foundational concept in toxicology,” says Lauren R. Teras, PhD, the senior scientific director of epidemiology research at the American Cancer Society, who is based in Decatur, Georgia. “How much, how often, and how long you are exposed all play an important role in potential health effects.”

What Are Pesticides, Exactly?

A pesticide is a substance used to kill, repel, or control pests. Within this category are herbicides that destroy weeds, insecticides that kill insects, fungicides that prevent mold and mildew from growing, disinfectants that target bacteria, and compounds that control mice and rats.

Some pesticides are more harmful than others. The World Health Organization (WHO) classifies the active ingredients in pesticides in the following categories:

  • Unlikely to present acute hazard in normal use
  • Slightly hazardous
  • Moderately hazardous
  • Highly hazardous
  • Extremely hazardous
Pesticides are classified according to “LD50," or the lethal dosage that would be needed to kill 50 percent of a rat population through oral or skin exposure. Pesticides in the “unlikely to present acute hazard in normal use” category would have to be used in high quantities to kill 50 percent of a rat population, while pesticides in the “extremely hazardous” category could be used in small quantities to get the same result.

People are exposed to pesticides daily, mostly by household products such as disinfectants, bug sprays, and weed killers. The health risks of pesticides largely depend on which ones you’re exposed to, how often you’re exposed, and how much you’re exposed to.

What the Science Says About Pesticides and Cancer

When we talk about pesticides and cancer, it’s important to distinguish between exposure to pesticides while working in agricultural settings and individual pesticide exposure from consuming fresh fruits and vegetables or using other common household products, experts say.

Increased Cancer Risks With Some Jobs

“Those at highest risk include agricultural workers, pesticide applicators, and landscapers who may handle these chemicals regularly over many years,” Dr. Teras says.

According to research that analyzed 62 studies, occupational exposure to pesticides increases the risk of cancer. The review included studies that evaluated indirect exposure to pesticides, direct exposure, and both. Participants in the studies could have been exposed to pesticides during the application process, they might have worked at a facility where pesticides are stored, or they may have come into contact with agricultural workers’ contaminated clothing and tools.

The authors of the report found many associations between occupational pesticide exposure and cancer. For example, exposure to dichlorodiphenyltrichloroethane (DDT) is linked to increased risk of prostate cancer, and exposure to lindane is linked to increased risk of non-Hodgkin lymphoma. Some pesticides have been linked to multiple myeloma, colorectal cancer, breast cancer, and cancers of the kidney, brain, and digestive system. It’s important to remember that people exposed to pesticides at work are typically exposed to high concentrations over several years, and that the studies don’t account for all substances participants may have been exposed to.

Pesticide Exposure on Fruits and Vegetables

Some studies have linked pesticide exposure from produce to cancer. “One recent abstract from the American Association for Cancer Research made headlines suggesting that fruits and vegetable intake was associated with lung cancer due to pesticide exposure,” says Urvi A. Shah, MD, a hematologist-oncologist at Memorial Sloan Kettering Cancer Center in New York City. But the study was limited in several key ways, she explains.

“The study results have very limited data to draw these conclusions, and several hypotheses need to be tested before these conclusions are drawn in this population,” Dr. Shah says. “It is a small sample size of 187 patients studied after their diagnosis and not followed prospectively.”

The results of the study shouldn’t dissuade people from eating conventional produce. “In the absence of clear data to link the cancer to pesticides, this study may confuse patients and the general population, making them avoid healthy foods. The conclusions should have been more speculative,” Shah says.

Ultimately, the research we have isn’t strong enough to suggest that nonorganic produce is bad for you, she adds. But we do know that eating fruits and vegetables decreases the risk of several cancers, including cancers of the mouth, larynx, pharynx, stomach, lung, and esophagus. And research suggests that by regularly eating these, you might also decrease your risk of cancers of the colon, prostate, and pancreas.

Risk From Household Products Is Minimal

The Environmental Protection Agency has banned many pesticide products over the year. In general, highly harmful pesticides are controlled for occupational use. Household pesticide products are generally much less harmful. As such, the risk of developing cancer from exposure to household pesticides is negligible.

“The general public can be exposed to pesticides in a variety of everyday places like homes, parks, offices, and schools,” Teras says. “For the general public, exposures are typically lower and more intermittent, but they can be more persistent depending on the chemical and the source of the exposure.”

These exposures can have an effect. Teras points to a study from the National Institutes of Health (NIH) as an example.

 “The study found that one common pesticide was detected in the blood of a high proportion of both farmers and nonfarmers, but at consistently higher levels among farmers, with recent occupational use being the strongest predictor of exposure levels,” she explains.

Experts stress that pesticide exposure through household products is not thought to meaningfully increase cancer risk. “It may be helpful to keep in mind that while low-level, infrequent exposures to pesticides are not thought to be major drivers of cancer risk, there's likely no completely risk-free option,” Teras says.

Should You Only Buy Organic?

Can you lower the risk of cancer by consuming organic products? The answer is no. Given the minimal risk of cancer from pesticide exposure in grocery and household products, you won’t lower your risk much by buying organic produce.

Food that is labeled “organic” has been grown and processed without the use of synthetic (man-made) fertilizers or pesticides.

 Both organic and nonorganic fruits and vegetables can contain small amounts of pesticides, but systems are in place to protect you from dangerous amounts.

“The Food Quality Protection Act passed in 1996 ensures pesticides used on food meet strict safety standards,” says Lindsey Wohlford, RD, a wellness dietitian at the University of Texas MD Anderson Cancer Center. The law limits the quantity of pesticides that can be used to grow food, which minimizes your exposure to pesticides, she adds. “In addition, the Environmental Protection Agency evaluates all pesticides, both new and existing, to ensure they are not harmful or being used in harmful amounts.”

It’s also important to note that “organic” doesn’t mean “pesticide-free.” Farmers can use natural or biological pesticides to grow organic food, Shah says. “Therefore, they are not pesticide-free.” Some organic pesticides are also toxic to humans; just because they’re organic doesn’t mean they’re “safe.”

We don’t have research suggesting that people who eat organic food instead of conventional produce have a decreased risk of developing cancer. For example, a systematic review and meta-analysis that evaluated 17 studies concluded that those who ate organic food didn’t have a lower risk of breast cancer, colorectal cancer, non-Hodgkin lymphoma, or overall cancer than those who consumed no or low amounts of organic food.

Every year, the Environmental Working Group publishes a list of 12 foods that contain the most pesticide residue and a list of 15 foods that contain the least pesticide residue (called the Dirty Dozen and the Clean Fifteen). “The Dirty Dozen and Clean Fifteen are helpful tools for consumers who want to reduce their exposure to pesticides,” Wohlford says. “[But] consuming a diet rich in a variety of fruits, vegetables, and other plant foods is most important. If organic produce is not affordable, do not avoid fruits and vegetables.”

Practical Ways to Lower Your Pesticide Exposure

Even though limiting pesticide exposure is unlikely to lower your cancer risk, it can lower your other health risks. For example, some pesticides have been linked to neurological problem such as Parkinson’s disease.

Pesticide use can also harm animals, fish and wildlife.

You don’t have to completely overhaul your lifestyle to reduce your exposure to pesticides. Taking some simple precautions can make a difference, experts say.

  • Clean your produce. Thoroughly wash all your produce under running water before eating it, Wohlford recommends.
  • Remove the skin. Peeling fruits and vegetables with a firm outer skin, such as carrots and apples, can also help, Wohlford says.
  • Don’t use an excessive amount of pesticides on your lawn or garden. “Use pesticides only when needed and apply the minimum amount necessary,” Teras says.
  • Follow the instructions on pesticide products you use. “This includes using recommended protective equipment and keeping children, pets, and others away from treated areas,” Teras says. You should also wash your hands after doing yardwork.
  • Don’t buy pesticides you don’t need. “It’s also best to buy only what you need — excess product can increase the risk of improper storage or disposal, which can lead to additional exposure,” Teras says.
  • Store fungicides, insecticides, disinfectants, and other products properly. “Keep pesticides stored safely and out of reach of children and pets,” Teras recommends.
