Treatment-Induced VMS: How They Differ From Natural Menopause

Treatment-induced VMS happens when medical treatments cause estrogen levels to drop quickly, disrupting the brain’s internal thermostat and making it harder for the body to regulate temperature. And these changes shouldn’t be mistaken for natural menopause.
Why Treatment-Induced VMS Feels Different From Natural Menopause
Natural menopause typically happens gradually. But when medical treatments trigger menopause, the body may not have the same opportunity to adjust to changes slowly. As a result, hot flashes and night sweats can feel more sudden and disruptive.
“The onset can be shocking, and the symptoms can be more severe,” says Alyssa Dweck, MD, a board-certified gynecologist practicing in Manhattan and the chief medical officer of Bonafide Health. “Even if it’s a perceived severity because there isn't a ramp up of mild symptoms, it can go from full on estrogen levels to depleted levels basically overnight.”
For people undergoing cancer treatments or other major health events, this sudden onset of VMS symptoms can add to an already heavy load. You might be coping with cancer recovery, surgery, or worries about your future health.
How Long Can Treatment-Induced Hot Flashes Last?
The duration of treatment-induced VMS often depends on why menopause occurred and whether ovarian function returns.
With treatment-induced menopause, symptoms can last just as long if your ovaries are surgically removed or do not bounce back. But these changes may also be temporary. For example, some people who experience menopause symptoms during chemotherapy may regain ovarian function and have their periods return after treatment ends, says Dr. Dweck.
“There is some predictability with certain treatments or surgeries, so treatment options can be offered to prevent symptoms from coming on suddenly or even starting at all,” she adds.
Treatment Options for Managing Treatment-Induced VMS
The best remedies for treatment-induced hot flashes and night sweats depend on several factors.
- Why menopause occurred
- Your overall health status
- Whether certain therapies are safe for you
Your gynecologist can help you weigh the benefits and risks of different approaches and create a treatment plan based on your individual needs.
Hormone Therapy
“For someone who was just diagnosed with breast cancer and is placed on chemotherapy, estrogen is probably not going to be something to consider,” says Dweck. “But for someone who, for example, has had both ovaries removed for a benign reason, estrogen given at the time of surgery may be all that’s needed to help prevent and manage symptoms.”
Nonhormonal Medications
For people who cannot — or choose not to — take hormone therapy, several nonhormonal options may help reduce hot flashes and night sweats.
Cognitive Behavioral Therapy
Treatment doesn’t always have to involve medication. Cognitive behavioral therapy (CBT) is an evidence-based approach that can help people manage the physical and emotional effects of VMS.
“In the right hands with the right therapist, CBT is fantastic for vasomotor symptoms,” says Dweck.
Lifestyle Strategies That May Help With VMS
Lifestyle changes may not completely eliminate treatment-induced hot flashes, but they may lessen their effects.
- Track your triggers. Keeping a symptom diary for a few months may help you identify possible triggers, like caffeine, alcohol, or spicy foods containing capsaicin, that can bring on or worsen hot flashes.
- Stick to a heart-healthy diet. A Mediterranean-style eating pattern that includes lean proteins, fruits and vegetables, whole grains, and healthy fats has been shown to reduce VMS. This style of eating can also help control blood sugar highs and lows, which may trigger hot flashes for some people, adds Dweck. Talk with your doctor to get the green light before making any diet changes or adding any herbs or supplements to your routine.
- Add strength training. One study found that postmenopausal individuals who perform a 45-minute resistance workout three times weekly experience half as many hot flashes after 15 weeks compared with those who remained inactive. Findings suggested that neurotransmitters released during weight training may assist the brain in regulating and stabilizing body temperature, helping reduce hot flash frequency.
- Prioritize sleep and stress management. Hot flashes and night sweats can disrupt sleep, which may affect mood, energy, and overall well-being. Stress-management strategies such as yoga, tai chi, mindfulness, or other relaxation practices may help some people better cope with symptoms.
Questions to Ask Your Doctor About Treatment-Induced VMS
When managing treatment-induced VMS, the most important question to ask yourself is how much your symptoms are affecting your daily life. Are they interfering with sleep, work, relationships, and your emotional well-being?
“What it really boils down to is whether it's distressing to you or interfering with your quality of life,” says Dweck.
If your answer is yes, it may be time to schedule an appointment with your doctor so you can come up with a plan that fits your health needs, preferences, and goals.
Consider asking:
- Are my symptoms likely to be temporary or permanent?
- What symptoms should I track before my next appointment?
- How do I know if my treatment-induced VMS is severe?
- Am I a candidate for hormone therapy?
- What nonhormonal options may help me?
- Are there prescription nonhormonal medications that can work for my symptom severity?
- How can we safely balance my primary medical treatment with effective options for VMS relief ?
- Are there specialized support groups, therapists, or clinics you recommend that understand treatment-induced menopause?
- What immediate coping strategies can I use for quick relief while we figure out a long-term plan?
