How Weight Loss Improves Metabolic Health in Type 2 Diabetes

If you have type 2 diabetes (T2D), your weight is more than just a number on the scale. For some people, even a little bit of weight loss can help improve metabolic health: the body’s ability to regulate blood sugar, burn calories, and manage blood pressure and cholesterol levels.

That’s because type 2 diabetes is closely tied to a condition called metabolic syndrome. With metabolic syndrome, the body becomes less responsive to insulin, which causes blood sugar levels to rise and increases the risk of complications like heart disease, kidney disease, nerve damage, and stroke.

Losing weight is one of the most effective ways to improve your metabolic health, with some research showing that losing just 5 to 10 percent of your body weight is enough to meaningfully improve your health.

What Is Metabolic Health?

There’s no set definition for metabolic health, but in general, “metabolic health is how well a person can convert the calories they take in to energy,” says Peminda K. Cabandugama, MD, a specialist in obesity medicine and endocrinology at Cleveland Clinic.

If your body has trouble using this energy due to a nutritional deficiency or too much (or little) calorie consumption, for example, you may develop a metabolic disorder such as metabolic syndrome — a cluster of interconnected risk factors that significantly raise the risk of heart disease, stroke, and type 2 diabetes.

In general, being metabolically healthy means you don’t have metabolic syndrome. Other signs of good metabolic health include normal levels of the following markers, without the use of medication:

  • Blood glucose
  • Blood pressure
  • Blood lipids
  • Waist circumference
  • Adiposity (body fat)

How Metabolic Health Impacts T2D

Type 2 diabetes doesn’t develop in isolation; it emerges from multiple metabolic dysfunctions, at the core of which is insulin resistance. When cells in the liver, skeletal muscle, and fat tissue stop responding efficiently to insulin, the pancreas compensates by producing more of it.

Over time, high levels of insulin can negatively impact other aspects of the body. For example, insulin sensitivity can slow down metabolism and raise triglycerides, HDL cholesterol, and blood pressure levels. Meanwhile, excess visceral fat — the fat that accumulates deep around the organs — releases inflammatory chemicals that make insulin resistance worse.

But because metabolic syndrome and type 2 diabetes share the same underlying cause (namely, insulin resistance), managing one condition can also help manage the other, which is where weight loss comes in.

How Losing Weight Can Improve Your Metabolic Health

Weight loss can improve insulin sensitivity by decreasing excess fat tissue.

Here’s why that’s important: Extra fat tissue releases inflammatory chemicals that cause insulin resistance. Weight loss, though, decreases that inflammation, says Silvana Obici, MD, the chief of the division of endocrinology at Stony Brook Medicine and the medical director of the Stony Brook University Hospital diabetes center in New York. If enough weight loss occurs, she adds, a person’s type 2 diabetes can go into remission, and their related conditions (such as high blood pressure) can also improve.

How to Get Started on a Weight Loss Plan

For people with type 2 diabetes, the most effective weight loss plans are usually gradual, sustainable, and focused on overall health rather than a rapid drop in pounds. This approach will look a little different for everyone. Consider working with a healthcare provider or registered dietitian to come up with a diet plan or treatment regimen.

Rather than aiming for dramatic weight loss, experts often recommend focusing on small, realistic changes that can be maintained over the long term.

Prioritize Protein and Fiber

Protein and fiber can help keep you feeling full and stabilize blood sugar levels. Good foods to try include beans, Greek yogurt, eggs, fish, vegetables, and whole grains.

Increase Physical Activity

Even without weight loss, exercise can improve insulin sensitivity. The general recommendation for adults is at least 150 minutes of moderate-intensity physical activity per week, along with muscle-strengthening activities at least twice weekly (just not on consecutive days).

Follow an Eating Plan That’s Right for You

A good meal plan will likely include more whole foods, including whole grains and nonstarchy vegetables, and fewer added sugars and refined grains. Know, too, that crash diets and highly restrictive eating plans can be difficult to maintain and may negatively affect blood sugar levels. A diet that’s more sustainable over the long term is likely to be more effective over time.

Talk to Your Doctor About Medications for Weight Loss

GLP-1 medications can also help people with type 2 diabetes lose weight. Drugs that have been approved by the U.S. Food and Drug Administration (FDA) include:

  • semaglutide (Ozempic)
  • tirzepatide (Mounjaro)
  • oral semaglutide (Rybelsus)
  • liraglutide (Victoza)
  • dulaglutide (Trulicity)
Talk to your healthcare provider to determine which, if any, of these medications is right for you. There may also be other options you can take, such as SGLT-2i inhibitors.

Is It Better to Focus on Metabolic Health or Weight Loss?

Ideally, you would focus on both, says Obici, who adds that each one helps improve the other.

“The evidence strongly suggests that metabolic health and weight loss are deeply interconnected and are not competing goals,” she says.

But some research also suggests that it may be more important to be metabolically healthy than to be at a certain weight. For example, a large meta-analysis found that metabolically unhealthy individuals at a healthy weight had higher mortality and higher heart health risks than those who were metabolically healthy but obese.

That said, weight management is still one of the primary treatments for type 2 diabetes and metabolic syndrome.

“In certain circumstances, it can lead to diabetes remission, a decreased risk of mortality, and better quality of life,” says Obici.
