Mild cognitive impairment (problems with thinking and memory) can get worse with certain lifestyle habits, like smoking, stress, and poor sleep. But by avoiding these triggers, you can improve your brain function and possibly prevent permanent cognitive decline.

New environments, crowded places, sleep apnea, untreated hearing and vision loss, and social isolation can also speed up changes in memory and thinking, says Manisha Parulekar, MD, the chief of geriatrics and a codirector of the Center for Memory Loss and Brain Health at Hackensack Meridian Neuroscience Institute at Hackensack University Medical Center in New Jersey.

Below, learn the most common triggers for worsening mild cognitive impairment (MCI) and how you can avoid them.

Lack of Sleep

You need sleep to think, process emotions, and interact with the people in your life.

“During sleep, the brain's glymphatic system — a waste-clearance network — removes toxic proteins, including amyloid-beta and tau,” says Mill Etienne, MD, MPH, a neurologist and a professor of neurology and medicine at New York Medical College in Valhalla, New York. “These are the same proteins that accumulate in the brain of people with Alzheimer’s dementia.”

“Sleep disruption can affect mood, focus, and memory, which already can undergo changes in early Alzheimer’s, leading to irritability, paranoia, anxiety, and confusion,” says Dr. Parulekar.

Dehydration

When you don’t drink enough fluid in the day, your brain can’t work as well.

“People with dementia may have decreased thirst sensation and may also forget to hydrate, which could increase the likelihood of dehydration,” says Dr. Etienne.

Low water intake also prevents your brain from clearing out toxic proteins, which affect cognitive health when they pile up, says Parulekar.

“For people with dementia, whose brains are already dealing with damaged neural pathways and reduced cognitive reserve, even mild dehydration can tip the balance from coping to struggling,” says Etienne.

Alcohol

When you drink alcohol, your brain cells can’t communicate as well as they normally do.

Alcohol also makes it harder for your brain to clean out harmful proteins, which can then build up and cause damage, says Parulekar.
One study of 235 people with MCI found that those who had about two alcoholic drinks per week had twice the risk of developing Alzheimer’s disease.

“Alcohol also can have a harmful impact on the chemical messengers in your brain, affecting your mood, behavior, and coordination,” says Parulekar. It also causes swelling and stress in the brain, which can lead to the death of brain cells, Parulekar says.

Smoking

Research suggests daily smoking actually shrinks your brain and prevents good communication between brain areas.

“Smoking causes your brain to lose tissue faster than it normally would as you age, which can lead to problems with memory and thinking,” says Parulekar.
The chemicals in cigarette smoke can make MCI worse by:

  • Promoting damaging protein buildup in your brain
  • Causing inflammation
  • Damaging or destroying brain cells

It also makes tiny blood vessels in your brain stiffer, which slows down blood flow and prevents enough oxygen from getting to brain cells, says Parulekar. “Smoking also increases the risk of other conditions that can lead to dementia, such as strokes and high blood pressure,” says Parulekar.

Stress

When you feel stressed, your body releases a chemical messenger called cortisol.

Research has connected high cortisol levels with worsened brain function, memory, executive functioning, language, and thinking speed, says Etienne.

“The hippocampus — the brain's memory hub and one of the first structures damaged in Alzheimer's — is especially vulnerable,” says Etienne. High cortisol can be toxic to your hippocampus, causing cell damage and harmful protein buildup, he says.

Older adults with high stress levels can have up to three times more risk for Alzheimer’s disease than those who report no stress.

Stress can also disrupt how your brain communicates, says Parulekar. “These disruptions can lead to changes in mood and behavior, such as aggression. This added stress can, in turn, make the disease's symptoms worse and speed up its progression.”

Overstimulating Environments

Loud, crowded, or unfamiliar places can cause sudden confusion and agitation in people with MCI.

“People with early Alzheimer's lose a critical cognitive ability: filtering,” says Etienne.

“The brain's ability to filter out irrelevant or redundant sound information in order to focus on important information may be impaired in individuals with Alzheimer's,” says Etienne. “For someone with dementia, this could mean that everyday noises, lights, or even textures become overwhelming; causing confusion, anxiety, and agitation that make daily tasks more challenging.”

This jump in confusion or behavior change can happen more often around bedtime — a phenomenon called “sundowning.”

This can be related to the level of sensory experiences you have over a busy day, says Parulekar. “As the day progresses, the brain becomes increasingly fatigued and less able to cope with environmental stressors.”

Urinary Tract Infections

Urinary tract infections (UTIs) can cause symptoms like worsened memory and other problems with thinking.

These brain changes can improve with treatment, says Etienne. But this trigger can be easily missed because the classic UTI symptoms (like burning and frequency) may not be present, he says.

How to Avoid Triggers

To avoid the triggers that make mild cognitive impairment worse, you first need to know which ones affect you the most. Keep a symptom diary, and when you or your family notice changes in thinking, memory, or behavior, you can record what you did that day or just before the change. After a while, you may see patterns that point to your triggers.

Once you know your triggers, you can take steps to avoid them. Parulekar recommends having a daily schedule, including a consistent bedtime and wake-up time. Parulekar and Etienne also suggest these tips:

  • Avoid alcohol and smoking.
  • Get some sunlight during the day.
  • Take part in activities, such as walking or going to a park with a caregiver.
  • Aim for seven to eight hours of consistent, uninterrupted sleep every night.
  • Add water-rich foods to your diet, like melon, oranges, berries, and tomatoes.
  • Set reminders to drink water in the morning, with each meal, and in the midafternoon.
  • De-stress with mindfulness practices like meditation and deep breathing.
  • Carry earplugs or noise-reducing earbuds for loud environments.
  • Schedule quiet recovery time after an outing or event.

When to Get Help

If you feel an increase in confusion or memory lapses, or your family says you’re acting strangely, tell your healthcare provider.

They can work with you and your loved ones to find treatment options and help you identify and manage triggers like a UTI.

“Sudden or rapid worsening of memory, increased confusion at night, hallucinations, or personality/behavioral changes should prompt medical attention,” says Etienne.
